Referral route
Referrals are from the Trusts Community Mental Health Teams and the Early Intervention Service. Assertive Outreach teams working outside of Sandwell can refer someone who is moving into, or returning to, Sandwell

Summary of services
Our aim is to support someone towards recovery and community integration. To do this we give someone assessment, treatment, rehabilitation, and psychological and social support. Practical hands-on help is central to our work. So too is helping our clients meet their aspirations in areas such as money, housing, education, and finding work. Each person using the service has a care co-ordinator. The care co-ordinator is responsible for organising the persons assessments and care. The care co-ordinator makes sure the person is getting the support that is right for them. Someone using the service also gets to know and trust all members of the team. We place a lot of importance on team members having regular face-to-face contact with the people using our service.

Time details
Monday to Friday 9am - 8pmWeekends and bank holidays 10am - 6pm. Between 9am and 5pm on weekdays the service can be contacted on the landline. Between 5pm and 8pm on weekdays, and on weekends and bank holidays, the service can be contacted on their duty mobiles.
