Who is the service for?
The Integrated Care Service provides assessment, treatment and management to patients 16 and over, who have problems or deterioration of their everyday functioning due to a physical problem. The service also provides rapid assessments to help prevent unneccesary admission to hospital.
Referral route
Clients can self refer to the Integrated Care Service or can be referred by their doctor or any other worker involved in their care.

Summary of services
The Integrated Care Service is a multi-disciplinary team, including: occupational therapists; physiotherapists; speech and language therapists; rehabilitation support workers; specialist nurses/therapists in Parkinson's disease, neurological disorders and multiple sclerosis; community matrons.
Time details
The call centre is open Monday to Friday 8am to 6pm.The Integrated Care Service operates seven days a week 8am to 8pm.
Remit
The aim of all of the Integrated Care Service is to support patients to regain or maintain their independence.
