- Visit patients who have a Sandwell GP and are housebound, have complex physical care needs or require short term intervention within their own home. The service promotes healthy lifestyles, physical, psychological and social well being and supports and encourages people with disability and long term conditions to live independent lives. The service provides cover 24 hours, 7 days a week.
