Summary of services
Sandwell Community Dementia Service comprises of nine local voluntary organisations to provide a single point of access for advice, information and support to those with worries about their memory or a dementia diagnosis, and their carers, across Sandwell.
The lead provider is Crossroads Caring for Carers and the partner organisations are: Agewell CIC, Dementia Pathfinders CIC, West Bromwich African Caribbean Resource Centre, Murray Hall Community Trust, St Albans Community Association, BUDS, Age UK Sandwell and Alzheimer’s Society.
We will work with you to seek a clear diagnosis of your condition and help you to make informed decisions about the support and care you receive. The support provided by Sandwell Community Dementia Service is free to access and will be tailored to your individual needs, to enhance your wellbeing and provide coping strategies to assist you and your family throughout the dementia journey.
